What is a DBQ?

A Disability Benefits Questionnaire (DBQ) is a standardized medical form the VA uses to document a condition's diagnosis, symptoms, severity and functional impact. If a service-connected condition has gotten worse, a DBQ shows its current severity in the format the VA looks for when evaluating an increase.

A DBQ documents your current severity. The VA makes all final rating decisions.

How It Works

Five clear steps from intake to a completed DBQ you can submit to the VA.

  1. 1

    Tell Us About Your Situation

Complete a short intake and tell us which conditions you're pursuing.

  2. 2

    Confirm Your Medical Evidence Needs

Our team reviews what you've shared and discusses which documents may be appropriate for your claim.

  3. 3

    Payment & Physician Scheduling

Once payment is complete, you're scheduled with the appropriate medical professional.

  4. 4

    Medical Evaluation

The physician independently reviews your records, conducts an evaluation when appropriate, and prepares your DBQ based on independent medical judgment.

  5. 5

    Quality Review & Delivery

We perform an administrative quality check and deliver your completed DBQ securely. You can submit it yourself, or use our courtesy VA fax submission where applicable.
